not insurance. These programs are elective wellness care packages designed to offer discounted
service rates for patients who choose to commit to ongoing care. They do not replace health insurance, cannot be
submitted to an insurance carrier for reimbursement, and do not meet any insurance coverage requirements.
Participation in these plans is voluntary. All plan terms, limitations, and expiration rules apply. Unused visits
expire at the end of the plan period and are forfeited. Plans are non-refundable and non-transferable.
